Thanks for raising this. It is true that patients may not be Singaporean and they don't have IC, but according to ICA, all foreigner in Singapore are uniquely identified by a "Foreign Identification Number" (FIN) which is similar in format to the NRIC number. So in this case, our system still allow foreigner to added to the system by using their FIN number.

Patients may not all be Singaporean. The patient may not have a Singapore IC, and may not have an IC in the format given in the UG. This will prevent foreigners from being added to the system, and foreigner are not uncommon.
