More optimised `Add` command

• Currently, the above are the only fields I could add for a patient.
• However, a nurse might want to add a note to the patient when he/she is adding the patient into the system.
• It would be nice if the nurse could just simple add notes when adding a patient (include a NOTES field) instead of needing to execute another command for it.

Thank you for your input. Adding more fields to the add command might increase the complexity associated with it, as there is now more fields one will have to keep track of. As such, this increase in complexity might not be a definitive improvement, and does not warrant a serious consideration. Although we could implement both methods of adding notes, the effort required to do so does not match the benefits of it in this current iteration. As such, we believe that this falls under the NotInScope category.
