Confusing formatting of patient notes

The patient notes are not arranged in a consistent manner, which make it impossible for the user to know which medication or remark corresponds to which previous appointment.

The screenshot below shows an example of when several previous appointments were created without remarks or medication, except for one of them. In the current GUI, is impossible to know which appointment the medication was given at, which will greatly frustrate the user.
