Better formatting of command summary table #447

Note from the teaching team: This bug was reported during the Part II (Evaluating Documents) stage of the PE. You may reject this bug if it is not related to the quality of documentation.

Perhaps a better way to format the command summary table is to split it into 3 sub headers (one for each mode) instead of having to write (in Patient/Visit/Prescription ie. .......) after every command
