Suggestion: make RFC002 compliant with the implementation, removing date of birth and adding prefix.
Reasoning: date of birth can be very intrusive because it relatively accurately identifies a unique citizen, while the combination of attributes only need to be unique within the care organization: the care organization is responsible that only care professionals can authenticate in their systems. This is a (temporary) shortcut until there are nationally available authentication means to identify care professionals.
