An attempt:

complication =def a pathological bodily process that occurs after a particular 
disease has been realized but is not itself a part of the disease's realization.

comment: complications can occur because an organism acquires a predisposition 
towards a pathological process during a disease course.

can anyone say this better?

This may work better as an instance-level relation between diseases

d1 'instance of' D1 ('is a' ogms:disease)
d2 'instance of' D2 ('is a' ogms:disease)

d2 'disease complication of' d1 at t2 = 
d1 'inheres in' b at t0
b 'has disposition' predisposition to D2 at t0 ('is a' ogms:predisposition to 
disease of type X)
realization of d1 occurs at t1
d2 'inheres in' b at t1

May not want to make acquisition of second disease (disposition) simultaneous 
with realization of first disease. Still the problem of causality here. Also, 
should this relation be built on underlying disorders?
