Change 'disease course' to 'disease realization' #63

As part of a discussion with Alan, Barry, and Werner, it was determined that 
'course' (and the relation 'course_of') will be a special technical term in BFO 
2.0.  As such, we should change 'disease course' to 'disease realization' (this 
will also improve clarity, I think).

It would be good to have a better justification than that (a link to the 
relevant discussion?). What would "disease course" in the new, BFO reserved 
sense mean that could be easily confused with the present "disease course"? 
"disease realization" sounds confusingly like the act of realizing that one has 
a disease while "disease course" is accepted terminology.

BFO 2 'history' will not apply to dependent continuants. Based on ogms-discuss 
thread discussion (see 
https://groups.google.com/forum/?fromgroups=#!topic/ogms-discuss/RaEexoNpYqs ) 
the label will remain the same and 'disease course' will be classified under 
BFO 2 process.
