Detection is on the part of a clinician, not a patient. The definitions of
clinical
history taking and physical examination clearly state that it is the clinician
carrying out these actions. The question is not when the patient presents to
the
clinician, but simply at what point the manifestation would be detectable had
the
patient presented at that time. If the stoic had shown up months earlier, the
manifestation may still have been detectable. The hypochondriac is a moot
point:
there is no manifestation present. Or rather, the manifestation is the claim
of a
symptom that does not exist.
Perhaps reword the definition to say "...prior to the time at which it would be
detected in a clinical history taking or physical examination, if the patient
were to
present to a clinician..." or some such.
