There is nothing in any of the definitions that restricts diseases to one 
disorder,
or disorders to one disease.  If two disorders cause the same disease, and that
disease has a "disease phenotype", then so be it: two disorders result in the 
same
disease phenotype, or rather, one disease phenotype may have any one of several
disorders as the physical basis of its underlying disease.  I see no problem 
for the
definition of disease phenotype.
