Fixes Ledger derivation paths #109

Model names are Ledger Nano S and X. Paths are segwit (BIP49) and native segwit (BIP84) Also minor markdown fixes

AFAIK this is new, older/no-up-to-date users would have their wallets set to 44. Maybe good if Ledger folks come make a PR for their New/Old settings.
