Apps->Call Center->Agents->Add should provide a choice for Type: #128

Similar to Issue 120 (http://code.google.com/p/fusionpbx/issues/detail?id=120)

What steps will reproduce the problem?
1. Go to Apps->Call Center->Agents.
2. Click "+" to add an agent.

What is the expected output? What do you see instead?
There should be a select for Type instead of text field filled by the default 
value ("callback") because there are only two agent types available (see 
http://wiki.freeswitch.org/wiki/Mod_callcenter#type).

What version of the product are you using? On what operating system?

In my experience uuid-standby is rarely used so a text box with a default value 
of callback and an option to change it to uuid-standby seems okay.
