Creation of a Credit Card renew page (or CC update) #69

When a CC expires or get blocked, the monthly donation fail and a message is received. We need to be able to orient that member to a " Credit Card renew page" or "CC update" that is not the "donation" page which include the SN3 and 4 goal. Maybe with same options. 10, 20, 50 and adding a "blank" monthly.

I started working on #75 and it seems like these two are somewhat related because both pages should show the same block of

<b> Choose a monthly amount:</b>
(select option) $10, $20, $50, $100, $___ (other)

and button that starts a stripe payment.

I had a couple of questions: 1) Should the CC update page include a make a one time donation option? 2) What should the button on the CC update page say? Update Donation? 3) Do we have context for how much they were donating before their CC expires? Should I add a query string that lets us auto-selects their previous donation amount (and maybe doesn't include lesser amount as pre-filled monthly options since they can always input them as other)?
