compare rocker (Ubuntu) vs. alpine base image

- with all CRAN packages, is a large OS actually still overhead?
- install R from source vs. install R from repos

How large are the images? What packages are not easy to install?

The question is how this handles system dependencies!

It doesn't; they'd have to be specified in the Docker file, before letting R install. DESCRIPTION files contain a SystemRequirements line, but these do not specify the debian package name.

Right - they ignore that. So #1 is probably a better place to start.
