I modified the TOC file in order to add another entry in the Table of Contents. 
The Table of Contents pane does not refresh/update, even when I hit save. Upon 
closing and reloading Sigil, the TOC has a few missing entries, and the last 
entry does not have an XHTML document tied to it. 

Additionally, I noticed that even after closing Sigil, there seems to be a 
runaway process running in Task Manager. Sigil.exe *32 takes up about 50% CPU 
(CPU 0 @ 30% and CPU 1 @ 80%), and another one of these processes is launched 
every time I reopen sigil.

What steps will reproduce the problem?
1. Open a epub book created in Calibre
2. Start editing chapters
3. Save after each chapter. (making sure you notice that the progress indicator 
comes up showing that its doing a cleanup)
4. Close Sigil.
5. Open Sigil and reopen the book you just edited.
6. Look though each chapter to see if your changes stuck.
7. Hopefully you will notice that Sigil did not save some changes to the 
chapters.

What is the expected output? 
The changes to the epub should stick.

What do you see instead?
I see a book with minor changes to the fisrt one or two chapters.

What version of the product are you using?
0.7.4 Portable Version

On what operating system?
Windows 7 x64

Im having a similar issue. I was editing a book that was created in Calibre. I 
added a footnotes section and started copying the footnotes from various 
chapters into the footnotes html document. I also was going though making text 
changes to each chapter making sure to save the Epub after making corrections 
to each chapter. When I went to  open the epub book I noticed that Sigil did 
not save my text changes. I was also editing one of the files outside the 
program and saving changes to that but it was just for one file.
