Update the Templates #24

We should look at cleaning up and simplifying the templates we use here at OASIS

What should the template look like and what sections are required.
Should we have annex and appendix?
How do we make our templates and document look and feel even more professional.
How can we make sure they work for text based formats like Markdown and/or AsciiDocs
Do we really need an HTML, PDF, and Word version?

We need to provide template options for work that is going to ISO or ITU or some other standards body. Often times those SDOs require the first several sections to be reserved for their boiler plate. Or the references need to be limited to A.5 certified organizations. Also, ISO requires that every reference listed be actually in the document. So I could see an easy to use and simple OASIS template and a version that could be used for sending work to ISO or ITU.
