This is a misunderstanding.
DMLex does not require each headword to be unique. DMLex says (in section 3.2) that the headword is one of the properties of entry which, together, determine the entry’s uniqueness (within the scope of the lexicographic resource).
DMLex does not require each definition to be unique. DMLex says (in section 3.5) that definitions are one of the properties which, together, determine the sense’s uniqueness (within the scope of the entry).
We do actually explain in section 1.3.5 that this is what the uniqueness constraints mean. The person who submitted this comment probably just missed that, hence the misunderstanding.

Is a unique definition or headword really required? I can foresee some use cases where this could be desired - can uniqueness be controlled by the entryID?
