updates for issues 350, 365 #435

This PR responds to ambiguities discussed in LS issues #350 and #365 as well as Architecture Spec issue 12 and MQTT Spec issue 20.

It adds:

Usage requirements for response results (in 3.3.2.2)
Example query features response for multiple profiles (new 4.2.3)
Corresponding update to history table (in Appendix D)
Correct typo in ToC for section 3.4.2.5

Language Schema changes may be needed to support the 3.3.2.2 changes and 4.2.3 example.
