The Markdown source includes a number of comments. These comments need to be converted to github issues and/or resolved.

Let's make sure to link the issue to a specific file for a specific commit/tag and line number.

As you can see I have made a start on this and intend to go through the whole document. When all comments are issues I will remove the comments from the working branch and submit a pull request.
