Rename README.rst to README.MD #270

Rename the readme extension from RST to MD so that GitHub interprets the readme as markdown and the markdown code, e.g. the Discord button and link, works as intended. Currently, the Discord link is broken since GitHub doesn't render the RST markups as required.
