Terms that involve GO + XAO #93

Consider using patterns that combine more general GO processes with XAO terms (e.g. abnormal GO:development + XAO:head) rather than GO-based patterns that include an anatomical aspect (abnormal GO:'head development') but don't reference an XAO term specifically. Consider obsoleting cases of the latter that are currently in XPO.

Many terms here should be obsoleted and be "replaced by" terms added in #94:
