We don't have "Provider Name" (e.g. "Beta Imaging Labs") available, so we use "Provider Role" instead (e.g. "Radiologist). We don't have "Patient Name" (e.g. "John Doe") available, so we use "Patient ID" instead (e.g. "Patient1"). Also, remove "Appointment Date" field as we don't have data for this, and switch row two and three, as per Nate's instructions.

Confirmed that this updated the UI as planned
