Current version is 1.2, adapted from OTN form.

Working on this over on https://github.com/mhpob/latitudes/tree/act if you'd like to assign me.

@jdpye some questions:

Do you know if any of the metadata submittal forms were created under any sort of license? It should be reflected here.
Is OTN the copyright holder?
Do you know who the original authors were?
