Added Data Consent Checkbox to Contact Form #222

Adding a data consent checkbox to the contact form. The form will not submit unless the consent is provided. Stylized the button more to show disabled versus active.

Still needed:

[x] Comment about closing data
[ ] ~Confirmation from legal about the direct marketing checkbox~

We were told we needed the privacy policy link and this covers it. Ready to go.
