Change the create-issue.yaml file located under the .github/workflows directory in the repository by copying the information in the file I have attached.

Parts to pay attention to:

First, completely delete the contents of the current create-issue.yaml file.
Then copy the content of the file I attached and paste it into the create-issue.yaml file.
Do not change the name of the file.
