Move includes to templates & fix missing category pages when config empty

Did two things in one here:

moved the template file(s) from includes to templates
fixed an issue where the plugin wouldn't generate index pages at all unless you explicitly defined them in the config (instead of using the site's categories instead)

Not needed anymore.. Plugin was abandoned
