Remove duplicated section of references. #1

I am happy with the Reference section now. References are duplicated in the gitbook output; not in the .pdf output (#9).

I am (for) now convinced that duplication in the gitbook output is ok. The duplication is in the output only, not the input. If changes are necessary, they happen in a single place.

The gitbook gets References at the end of each chapter and at the end of the book. Saving space is not a problem of html format, so there is no reason to avoid having references close to where the reader needs them and also at the end, for those who want just that.

The .pdf is a different story. Space is limited; It must be saved because .pdf is optimized for printing.
