Create new Primaries map #87

Things to do:

[x] Work out Output Area to Primary connections using customer postcodes
[x] Build Primary geographies up from Output Areas
[x] Identify unassigned Output Areas and manually assign them to Primaries
[x] Build "cut outs" for Direct to Supply Point connections
[x] Combine cut outs and Primaries into one map

This is being done in the primary geographies repo (private for now).
