@anibalsolon - I don't have a write access so I had to create a PR

I've removed to submission checklist, since I understand that adding the field required=true should be enough. Let me know if you think this still might be useful

(btw. I still haven't changed the guide, but will work on this)

@anibalsolon - sorry for the errors, I opened this in "full screen" and couldn't find the preview option...

@djarecka no worries! since you were not able to work directly in the main branch, you couldnt see the validations that Github does when you commit it. They dont have a preview for issue forms, unfortunately (it shows blank)
