Refer to cohorts using their id instead of name (which is not available anymore)

A better fix would be to add in all the shorthand names again. We should add these short names to the master excel on Teams and then rename the cohort files (json and sql). The latter could cause issues in other functions.

@keesvanbochove Good catch. I have now also updated other usages of cohortsToCreate$name.
