package rename and naming cleanup #249

Packages should be renamed to org.ohmage ...

Old references to AndWellness should be renamed to ohmage.

The readme for this repository also needs to be rewritten.

Also, code needs to be reformatted according to the readme rules.

I'm also switching "Aw" namespace references to "Oh"

Not changing LoginViewAndWellnessImpl to LoginViewOhmageImpl because LoginViewOhmageImpl already exists.

also not reassigning the appconfig name to "ohmage" from "andwellness" because of possible conflict
