Refactor clinical information for annotation into a separate collection #46

This makes the upgrading process much simpler, as we can then upload all the genomic information without troubling the existing clinical annotations.

Actually, I've ended up refactoring some other annotation into a separate collection, like Wikipedia annotation too. This is a good base to build on for this, as it also enables references to be found and matched.
