Add annotation logging and digital signing #52

We don't audit or log the annotations, which we used to do in GPS a long time back. This was actually useful to have, and it's easy, so we should do it again. A sign-off process would also be handy, but in many ways digitally signing the PDFs would be more appropriate. We might be able to integrate that into the rendering process.
