As suggested in issue #103, this PR changes the site directory structure to bring back the old patterns directory.

Tasks done in the two commits:

[X] Move the list of patterns as per https://github.com/okfn/handbook/tree/ae934ab2efac136979a3765cf01ffae96fbc0e74/pattern from /data-guides/ to /data-guide/patterns/
[X] Rename directory /data-guides/ to /data/
[X] Rewrite the following files: index.md - copied from the previous version above.

Looks good.

Couple of minor meta points:

Please use #103 rather than [ISS103] going forward as github recognizes the former but not the latter.
Your first commit message is a bit cryptic.

I used the markdown symbols in the message and shouldn't. I think I can edit the commit message.
