Getting 'access denied' unexpectedly #3

Hi, I'm getting an "access denied" error and I'm wondering if you can give me some clues. I'm confident Okta is setup correctly, but maybe there is something in Kong that I missed?

I am redirected to Okta to login, but on the redirect with the code is when I see the following error:

One additional note that may help: I did not see the groups claim as outlined in the docs. I had to add it in.
