EMTV issues #91

Your fixing the "titles" and "notes with verse Ref" so that EMTV will display 
in the And Bible properly.

Titles = <title></title> 
at the end of the verse 
<verse osisID="Matt.2.12">And having been divinely instructed in a dream not to 
return to Herod, they withdrew into their own country by another 
way.<lb/><lb/><title>The Escape to Egypt</title></verse>
In And bible this title is moved to after verse 11, it needs to stay after 
verse 12.

And before verse 1
<chapter osisID="Matt.2">
<title>The Vision of the Wise Men</title>
<verse osisID="Matt.2.1"></verse>

Notes = <note>text<reference osisRef="2 Cor.11.2">2 Cor 
11:2</reference>text</note>
So that one "note" will display both text and verse Ref. properly.

I'm changing the WoC quotes so that OSISqToTick=false is not needed, right now 
there are double quotes for the WoC in the And Bible and BibleDesktop.
