Add optional terms and conditions checkbox when applying for an account #15

This plugin is currently run on our demo server. We have an SLA but it's not presented to people filling in the form. We should add a link or embed the SLA, and require a checkbox to be ticked.

In particular https://github.com/ome/prod-playbooks/issues/197#issuecomment-523401649, I agree we don't necessarily want to have the app hosting the SLA itself to support deployments without omero-signup but with their own SLA e.g. in our case, outreach, nightshade, learning, idr.

An optional checkbox with an hyperlink to a static page would probably cover most of our use cases.
