Fixes #7 by adding a link to the iOS Settings when disabled, and generally improves the HomeScreen layout for all device types.
Unfortunately we can't link directly to the Safari > Extensions settings due to privacy constraints from Apple, but at least we can open the Settings app.
Also makes the logo green when enabled; user feedback indicated "red means bad".
