[ ] check practitioner if created on keycloak
[ ] check if the assigned practitioner on HAPI is more than 1
[ ] Identify the correct practitioner as provided in the csv
[ ] clean up the other practitioner by deleting their group and practitioner role

@ageryck This might have effects if the assignment CSVs have already been run.

We need to also check the assignments to try to identify the correct practitioner.

@dubdabasoduba the assumption is that the same uuid provided during creation for the practitioner is the same uuid provided during assignment. This is the uuid that we will keep, as we delete everything else. Does that sound okay?
