Fix size selection #25

Was always using default instance type as;

  1. Instance specific types were being looked up with environment rather than instance_type
  2. CLI overrides using --size weren't being passed through

could we have a test for this?

@davidszotten - Added tests, however they may conflict with the replicated functionality on the cfn_support branch.

This should only be merged if we want to release this before the cloudformation stuff.
