More updates for CMI-PB demo #37

Currently this branch is needed to run the demo from here: https://github.com/jamesaoverton/cmi-pb-terminology/tree/next-2

I'm going to keep this as a draft since I'll probably need to add more to it.

@jamesaoverton this is a pretty major PR with a lot of changes, but I confirmed that sprocket still works with SQLite databases & Swagger endpoints, as described in the README. It would be good to have this merged in if we want to make a release of sprocket, as this is what's driving nanobot.
