Hides dynamically created TOCs for examples and microservices #233

Modifies the maketoc.sh to include the :hidden: parameter so that the entries remain in the lefthand site-nav, but no longer appear in the contents of their respective pages.

Modifications will be in examples.txt and microservices.txt (included in index.rst of each directory).
