issue with medical prescriptions #177

if you submit 2+ files same prescriptor and date you get just one file displayed. and before you ask , yes It's possible to have multiple medical prescriptions on same date for one patient. Steps to reproduce: select patient > add 2 descriptions with each same doc and same date > click on the file uploaded

I tried it through the medical prescription interface , and there is no error.

Even from the patient interface it is still ok
