The file should be self-documenting (like Sphinx, etc.).

Add a step to #29 to update the template.

In retrospect, there is too much content for the file to self-document (e.g. what does a "threshold" mean without the context of the methodology?). So, I just included links to the documentation. (Sphinx moved to this approach, too.)
