Align schema and codelist patching logic with CoVE #25

CoVE might do some things that ProfileBuilder doesn't (yet).

Ideally, all projects would use the same library.

With #27, I think this library now has feature parity with CoVE (minus less error handling/reporting).

CoVE can use this library if that makes sense. That said, there's no more work in this issue.
