Add README-template with info on submitting issues #8

Now that we decided not to allow issues in individual extension repos and rather collect all of them here https://github.com/open-contracting/ocds-extensions

Once this is merged, we have to go adding README-template.md text to every extension repo.

For extensions managed by OCP, we want issues reported to that repository, but if other people create and publish their own local or community extensions, they can choose to have their issues reported on their own repositories, right? I think we may just want to have the readme template in the handbook for our own use. What do you think?

@jpmckinney you are right, I was missing community extensions in the picture! Yeah, I will transfer this info to the handbook, adding some notes for community vs core for issues.
