Add CONTRIBUTING and CODE_OF_CONDUCT files.

Humble beg to avoid the "Contributor covenant" style of code of conduct - you know - the ones with threatening semi-legal language that looks like it has been written by a ChatGPT HR department. We (Nibabel) went for : https://github.com/nipy/nibabel/blob/master/.github/CODE_OF_CONDUCT.md . I (Matthew) spent some time discussing and editing the Scipy code of conduct - https://github.com/scipy/scipy/blob/main/doc/source/dev/conduct/code_of_conduct.rst - although I still think that is too far towards the pseudo-legal / HR end of the spectrum.
