This PR removes the DL3 master index table and improves the description of the HDU and obs index tables a bit.
This was discussed extensively in the past years and in #7.
Almost no-one except me is pro index tables, so removing the master index table should be welcome to most.
Note that we removed support for it in Gammapy, @jknodlseder doesn't plan to continue to support it in ctools (see https://github.com/open-gamma-ray-astro/gamma-astro-data-formats/issues/7#issuecomment-340934748) and it's not used much. It was only used in HESS a few years ago, but we stopped to use it.
I'll merge this tomorrow morning. Comments welcome.
