Use modern/official doxygen layout with treeview, remove OMPI style from doc pages #160

doxygen has a useful "treeview" mode that makes HTML navigation much easier (dynamic sidebar). But integrating that within the OMPI theme would be hard. We should use official doxygen style, headers (generated by doxygen -w), stop embedding doc pages in the OMPI style, and just add backlinks from the doc to the main hwloc page.
