Bug: Post a highlight form has no clear labels

Describe the bug

The "post a highlight" form has no clear labels on the inputs, only placeholders. This hurts form accessibility.

I think this makes a good first issue :)

Steps to reproduce

1. Go to highlights tab
2. Click on the "post a highlight" input to open the form
3. Notice that the text area and 2 input fields have no labels
