[infra] Page feedback create-issue link should include page title, URL path, and source URL, if possible #5091

In the latter thread I wrote:

There is no simple way to address this. I’d have to write some custom code. The feedback-form feature was designed quite a while ago and there quite a few aspects of the implementation that I don’t like. As @Severin points out, it is an upstream issue, but I’m not sure when I’ll be able to address this in Docsy, because it isn’t obvious how to provide the functionality OTel needs in a manner that is generally useful for all/most Docsy-based projects.

That being said, I’ll see what I can hack together when I have the time.

Our teammate who handles infrastructure for the project is OOO at the moment, so I would suggest waiting to start on this until he returns and can respond to you himself. In the meantime, please have a look at our other open issues to see if there's documentation you'd like to contribute to.
